We compared the activity of dicloxacillin with that vancomycin against 15 oxacillin-susceptible, methicillin-resistant Staphylococcus aureus (OS-MRSA) clinical isolates. We describe a case of Henoch-Schoenlein purpura, associated with oliguric acute renal failure due to interstitial nephritis histologically proven, in patient staphylococcal chest infection. The clinical syndrome appeared twice after vancomycin administration and yielded on the withdrawal drug. As pathogenetic mechanism we propose an anaphylactoid reaction either or glycoprotein released breakdown microbial cells caused by vancomycin.
