The pathogenesis of biliary atresia (BA) is propounded to be an immunological process, but the detailed mechanisms remain unclear. In recent studies with abnormal findings, T cells have drawn attention in mechanism BA pathogenesis. Furthermore, cytomegalovirus (CMV) infection patients resulted elevated levels helper (Th1 and Th17) reduction regulatory cells. Thus, CMV considered as a cause BA. 症例は31歳,男性.前頸部腫脹,発熱,動悸で来院した.超音波検査は亜急性甲状腺炎を支持せず,甲状腺自己抗体は陽性を示した. 123Iシンチグラムでは甲状腺への取り込みが見られず,バセドウ病は否定的であった.本例では自己免疫性甲状腺疾患を基礎に破壊性の機序が加わり,さらに経過中に甲状腺受容体阻害抗体が高値となり,甲状腺機能の低下が導かれたと考えられた.
