Delayed rupture of intracranial aneurysms after endovascular treatment is a rare but serious complication. We report the first documented case late aneurysmal following with Contour intrasaccular device. A patient in their 60s basilar tip aneurysm underwent using 14-mm Fifteen months later, presented fatal intraventricular hemorrhage, and imaging revealed device displacement growth. This underscores importance meticulous sizing follow-up, especially for large aneurysms.
