歯科医療現場における齲蝕の診断は困難を伴い,病変の深さや拡がりを正確に行う非侵襲的な画像表示方法の開発が望まれている.歯科用X線写真の初期齲蝕に対する感度は低く,透過像として検出できるのは象牙質内部まで深く進行している病変であることが多い.本研究は光干渉断層計(OCT)を用いた齲蝕の診断精度の検証を目的とし,in vitroおよびin vivo実験を行った.平滑面齲蝕,咬合面齲蝕,隣接面齲蝕ならびにコンポジットレジン修復の窩底部の齲蝕に対するSS-OCTの感度,特異度,受信者動作特性曲線の面積を計測し,視診またはX線写真の結果と比較した.SS-OCTの断層画像では,齲蝕病変部を輝度の上昇によって表示することができた.厚さ2 mmまでのコンポジットレジン修復窩底部の齲蝕も明るく表示され,健全な窩底象牙質と区別することができた.
