ゲノム解析技術の進歩を背景に,包括的がんゲノムプロファイリング検査(CGP検査)が実臨床に導入された.これら「プレシジョンメディスン」はがん薬物治療の個別化への道を開く一方で,検査に耐えうる十分量の検体採取が困難な例も存在する.膵胆道癌におけるEUS-FNA/Bでは,デバイスを含めたアプローチの選択や穿刺回数の増加,病理部門との連携による標本作製の工夫により,CGP検査の成功率を高めることも可能である.また近年,EUS-FNA/B検体を用いた核酸解析に加えて蛋白発現の評価を行うことで膵胆道癌診断能の向上を図るなど,多角的な分子診断を目指す取り組みが報告されている.病理診断の補完のみならず,病勢把握や再発予測,新規治療法の探索など,今後のゲノム診療がカバーする領域は多岐にわたることが想定され,さらなる発展が期待される.

膵癌のゲノム解析では,4つの遺伝子異常(KRAS,CDKN2A,TP53,SMAD4変異)を高率に認める.最近の研究により,ゲノム,遺伝子発現,タンパク,代謝などの様々なレベルでの異常が明らかとなり,これらのプロファイリングによる個々の患者の発癌や進行パターン,治療効果予測に応用されることが期待される.2019年に適切な薬物治療の提供を目的とした遺伝子パネル検査が保険収載され,本格的なゲノム医療の時代を迎えた.このような新しい診断技術を早期膵癌の発見や遺伝素因など高い発癌リスクを有する人々の発病予防を目指した医療へと拡大するには,多様な分子異常の検出方法の確立が求められる.これら膵癌の分子診断には,膵内の多発病変の存在と腫瘍内の不均一性,癌のクローン進化の理解が重要となる.本稿では,膵癌の発生過程でみられる分子異常を概説し,診療への活用が期待される最新の技術革新について紹介する.
