We describe the technique for first robotically assisted bilateral bronchoplasty treatment of tracheobronchomalacia. Since 1954, this disease process has been treated with a right thoracotomy and posterior splinting tracheal membrane mesh to restore C-shape trachea. Traditional video-assisted thoracoscopic surgery limited benefit in these cases owing technical challenges. We present a unique case of patient with clinical history distal esophageal carcinoma and CT appearance lipomatous hypertrophy the interatrial septum who displayed fluorine-18-fluorodeoxyglucose (FDG) uptake in septal fat on only second 4 serial PET/CT studies. As scans are used for initial staging carcinoma, one should not mistake more benign causes mediastinal metastasis when observing an intermittent pattern scan.
