症例は47歳男性.明らかなアスベスト曝露歴は有さず,19歳時にHodgkinリンパ腫に罹患し,化学療法および胸部への放射線治療(マントル照射)を受け,寛解していた.発熱,胸痛,呼吸苦を契機に右胸水貯留を指摘され,精査の結果,胸膜中皮腫(上皮様)の診断に至った.集学的治療の適応と判断し,胸膜切除/肺剥皮術と化学療法による集学的治療が施行された.食道との癒着が高度で,術後に食道胸腔瘻による膿胸を発症したため,大網充填術を行った.術後5年目に同側胸腔内に局所再発をきたしたものの薬物療法により消失し術後7年が経過するが生存中である.過去の放射線治療によって発症したと思われた胸膜中皮腫の一例を経験した.アスベスト曝露歴を有さない胸膜中皮腫患者においては胸部放射線治療の既往の聴取が重要である.また手術を行う際には縦隔への放射線治療による高度な癒着が懸念されるため注意が必要である.
