We report a case of broncho-mediastinal fistula in 73-year-old man who presented with fever and hoarseness. Chest computed tomography showed 35 × 45-mm mass the left hilum an adjacent cavity measuring 60 25 mm. He was diagnosed as having squamous cell carcinoma lung on basis sputum cytology. 肺内伏針は,肺化膿症や膿胸などの感染症を合併する可能性や有害物質を含有している可能性があることから発見次第,早期の外科的摘出術が望ましいとされる.今回,我々は長期間にわたり体内に存在していた伏針を透視を併用した胸腔鏡下肺部分切除術で摘出した.症例は63歳男性,25年前に胸部の伏針を指摘されたが放置していた.他科術前検査にて胸腔内迷入針を疑われ,当科紹介となった.針の侵入経路は不明だが,幼少時に家業として縫製業が営まれており,経胸壁侵入を疑った.透視で確認しながら,胸腔鏡下にて異物を含む縦隔脂肪織と一塊にして右肺部分切除術を施行した.伏針は腐食脆弱化し,異物のみの摘出術は困難であったため,周囲組織との一括切除を必要とした.その際に術中透視は異物の同定と完全切除に有効であった.
