症例は47歳男性で, 既往歴は15歳頃に糖原病Ia型と診断され, その頃より心肥大, 高血圧, 痛風腎, 肝腎障害あり. 家族歴は両親同士は血族結婚であり, 弟も糖原病Ia型である. 現病歴は平成8年12月23日より全身倦怠感あり, 12月25日救急車で来院. 高度の腎不全による高カリウム血症を認めたのでHDに導入した. 12月27日にCPK, GOT, GPT, LDHおよび血中ミオグロビンが上昇したので横紋筋融解症と診断しCHDFに変更した. さらに横紋筋融解症よりDICを併発したのでCHDFとともにDICの治療やビリルビン吸着等を行った. しだいに改善したので慢性維持透析に移行し経過良好である. glucose-6-phosphataseの遺伝子解析は, エクソン5のdirect sequence法を用いてG727 T変異のホモ接合体と診断された. 本症例のように糖原病Ia型が直接横紋筋融解症の誘因になったという報告は見当たらない. 乳酸値が高値を示していたことより, 糖原病Ia型の存在のため乳酸アシドーシスをきたしやすい状態にあり, 横紋筋融解症を惹起したと考えられた.
