症例は全身性エリテマトーデス(SLE)にループス腎炎と抗リン脂質抗体症候群(APS)を合併し26歳で血液透析導入となった31歳女性.19歳よりAPSに対して抗凝固薬を開始したがその後肺胞出血を合併したため内服を中止していた.SLEとAPSの病勢が落ち着いたことを確認し31歳で母親をドナーとする血液型不適合生体腎移植を施行した.APS合併レシピエントに対する生体腎移植では血栓症合併リスクが高く,抗血栓療法を行うことが一般的である.本症例では手術7日前から抗血栓療法を開始し術後は血栓症を含め大きな合併症なく経過した.出血合併症のリスクを考慮し術後67日目に抗血栓療法を終了した.しかし術後150日経過した頃から左上肢シャント部より末梢の血流途絶を認め,血栓症として抗血栓療法を再開した.APS合併レシピエントにおける血栓症発症リスクの層別化や抗凝固薬の終了時期に関するコンセンサスは得られておらず今後検討が必要である.
