患者は58歳女性.IIIC期卵巣癌に対する腫瘍減量手術後に化学療法が施行された.最終投与から5年後の胸部CTで剣状突起後方の前縦隔腫瘍の腫大を指摘された.PET-CTで径2.5 cmの腫大した同病変にFDGの高集積が認められたが,他に明らかな集積なく,孤立性のリンパ節転移が疑われた.化学療法終了後から長期間経過後の単発病変であり,診断・治療目的に単孔式胸腔鏡下摘出術が施行された.鏡視下で病変を指摘できず,病変を含む心膜脂肪ごと心膜,横隔膜から剥離,摘出された.病理診断は卵巣癌(高異型度漿液性癌)のリンパ節転移であった.現在,維持療法継続中でリンパ節摘出術後3年無増悪生存である.卵巣癌では横隔膜を介したリンパ路から前縦隔リンパ節転移を起こすことが知られている.卵巣癌において遠隔転移となる前縦隔リンパ節転移であるが,呼吸器外科医も留意すべきリンパ節と転移経路と考え,文献的考察を加えて報告する.

患者は30歳男性.フットサルのプレー中に呼吸困難と右胸痛を自覚したため当院を受診した.右特発性血気胸の診断で胸腔ドレナージが行われ,血性胸水が認められた.持続性の血性排液であったため,緊急手術の方針となった.搬入途中でショック状態を呈したため,細胞外液の急速投与が行われた.手術は右第6肋間中腋窩線部に挿入された胸腔ドレーン刺入部を利用した単孔式胸腔鏡手術で行われた.胸腔内に血腫と胸腔頂部壁側胸膜面に血管様索状物が認められ,同索状物が出血源と同定された.手術開始後約8分で,同部に対してバイポーラによる焼灼止血が行われた.血腫除去後,肺尖部の破裂ブラの切除が行われ,手術終了となった.手術時間63分,術中出血量41 ml.術後経過良好で,術後3日目に胸腔ドレーンを抜去,入院8日目に退院となった.胸腔ドレーン刺入部を利用した単孔式胸腔鏡手術は速やかに胸腔内の観察・止血を容易に行える点で有用である.

背景.肺癌肉腫は癌腫と肉腫の混在する稀な悪性腫瘍で,進行が早く予後不良とされる.本例は肺癌肉腫に対し診断時から遡ってCT像を観察し得た貴重な症例のため報告する.症例.72歳女性.腹部大動脈瘤に対する人工血管置換術の術後感染のため開放ドレナージが行われた.ドレナージ後2ヶ月のCTで右肺中葉に内部不均一な低吸収域を示す径2.6 cmの結節影を指摘され,当科紹介となった.後方視的にみた7ヶ月前のCTで同部に径0.5 cmの結節影を認めた.経気管支生検施行も微小検体のため良悪性判定が困難であった.生検後のCTで径4.0 cmと増大を認めたため再生検を行った.異型の強い紡錘形細胞を認め,紡錘形細胞腫瘍の診断で単孔式胸腔鏡手術施行した.術中所見で下葉浸潤が疑われ,右肺中下葉切除術施行とした.病理所見で径5.0 cmの腫瘍は悪性末梢神経鞘腫瘍と骨肉腫と軟骨肉腫が混在し,扁平上皮癌成分も認めたことから癌肉腫と診断,病期はpT2bN0M0,stage IIAであった.術後早期に再発し,術後6ヶ月で原病死した.観察期間での腫瘍倍加時間は23日であった.結論.本例の急速な腫瘍増大は組織型を反映したものかもしれない.
