自記式「記憶の質問紙」 (患者用132問,家族用104問) を作成して,健忘患者とその家族に施行し,健忘症状に対する患者の自覚度の障害 (unawareness (UA)) を, (1) 疾患・病巣側性別, (2) 神経心理学的検査成績との相関に焦点をあて,家族の患者の健忘症状に対する評価得点から患者の健忘症状の自己評価得点を引いた値を UA 値として検討した。 対象は54例で,疾患別では,側頭葉てんかん20例,脳血管障害14例, Alzheimer型痴呆 (DAT) 16例,まだ DAT と診断できない変性脳萎縮疾患4例,病巣側性別では,左半球病巣11例,右半球病巣11例,両側半球・び漫性病巣・病巣不明 (BHL) 32例である。 値は,疾患・病巣側性別で有意差 ( と BHL で高値) があり,また右半球損傷を示唆する非言語性記憶検査成績低下などと有意な相関がみられた。したがって, unawarenessは右半球損傷と関連があり,さらに左半球病巣が加わるとより重度となると考えられた。

QT延長症候群の既往のある86歳男性,突然倒れこみ救急要請された.橈骨動脈は触知可能であったが,JCS III-200の意識障害と右共同偏視,左上下肢の麻痺を認め,脳卒中疑いで脳神経外科へ搬送.MRI撮影後 JCS II-20,左麻痺も改善.DWIで信号変化はなかったが,右MCA領域にFLAIR vascular hyperintensityを認め,MRAでは右MCAは描出不良であった.脱水があり,当初はこれにより血行力学性に右MCA領域の虚血を来した可能性が考えられた.入院後,心室期外収縮が頻発し,トルサード・ド・ポワント型心室頻拍(VT)が出現した.初回発症時の症状は,VTによる脳虚血症状であったと考えられた.VTによる脳虚血発作は,来院時消失していることがあり,来院時の検査結果で原因が判明しにくい.詳細な病歴聴取が重要であり,特にQT延長症候群の既往がある場合には,VTの可能性を念頭に置く必要がある.
