症例は69歳の女性で,肛門痛を主訴に当院を受診した.肛門周囲に全周性の発赤とびらんを認め,下部消化管内視鏡検査で肛門管内に隆起性病変を認めた.肛門皮膚のびらんからの生検にて印環細胞癌と診断され,Paget現象を伴う原発性肛門管癌と考えた.造影CTにて右鼠径部にリンパ節の腫大を認め,経会陰内視鏡アプローチ併用腹腔鏡下直腸切断術,右鼠径リンパ節郭清を施行した.病理組織学検査では肛門皮下に異型上皮細胞の充実胞巣と一部に中分化型腺癌を認め,表皮内に印環細胞様形態の癌細胞が進展していた.免疫組織学的染色では皮下の癌胞巣と表皮内の進展部位にCK7,CK20,CDX2,synaptophysinが陽性で,GCDFP-15,GATA3が陰性であったこと,また腺癌成分が20%程度であったことから,Paget現象を伴う肛門管原発神経内分泌癌と診断した.術後3か月で肝転移再発を認め,1年5か月で癌死した.
