Sulfamethoxazole/Trimethoprim (SMX/TMP) is a commonly used antibiotic, its' known adverse reaction of hepatotoxicity leading to acute liver failure considered rare. We present case 22 year old female who developed jaundice and one week after taking SMX/TMP for UTI. After an extensive work up, clinical diagnosis induced was reached. Over the course several weeks she made good biochemical recovery with supportive care. In this report we describe her presentation course, brief review literature.

Brugada type 1 ECG pattern is the hallmark for diagnosis of syndrome which a cause sudden death due to ventricular arrhythmias. We present case previously healthy young man who was admitted with productive cough greenish phlegm and right-sided chest pain subsequently diagnosed as acute pneumonia. Alternating bundle branch block (ABBB) is a less commonly encountered phenomenon with the advent of re-perfusion therapy for acute myocardial infarction. ECGs simulating appearance an ABBB need to be carefully analysed. We present ECG showing baseline Left Bundle Branch Block(LBBB) progressing high grade AV escape complexes having Right Block (RBBB) morphology. Such can mistaken if not analysed carefully.
