Nevoid basal cell carcinoma syndrome (NBCCS), also known as Gorlin syndrome, is a rare hereditary condition characterized by wide range of developmental abnormalities and predisposition to neoplasms. 症例は29歳，男性。8ヵ月前から生じた右大腿内側の粘液型脂肪肉腫に対して，手術目的に当科に入院した。術前に施行したCTでは薄筋に接して内部の均一な，楕円形の腫瘤を認めた。MRIではT1強調画像で均一な低信号，T2強調像で均一な高信号を認めた。エコーでは境界明瞭で全域に血流の豊富な腫瘤を認め，内部は大小の隔壁で区切られていた。エラストグラフィでは辺縁部で硬く，内部で柔らかい像を認めた。腫瘍細胞は多数の隔壁で区切られており，周囲との境界は明瞭であった。病理所見では大量の粘液基質や，印環細胞様の細胞を認め，chicken－wire様の分枝を伴う毛細血管が多数認められた。好塩基性の沈着物を認める領域ではアルシアンブルー染色が濃染した。自験例は臨床的，病理学的にも典型的な粘液型脂肪肉腫と考えられた。
著者らは, 血液透析患者に認められる起立性低血圧の成因が, 交感神経, 副交感神経のいずれの自律神経障害に起因するかを明確にする目的で種々の自律神経機能検査を施行し, その検査成績をもとに起立性低血圧を有する患者の看護についても検討を加えた.対象は12症例 (男4例, 女8例) で, 年齢は25歳から75歳, 平均54.1±14.0歳である. 透析歴は平均39.4±49.7か月で, 原因疾患は慢性糸球体腎炎4症例, 糖尿病性腎症6症例, および原疾患不明2症例である. 自律神経機能検査として立位負荷試験, 寒冷昇圧試験, アドレナリン負荷試験, 硫酸アトロピン負荷試験を施行した. 立位負荷試験では起立性低血圧が認められた群7例 (Group 1) と認められない群5例 2) に大別された. Group 1では, 5例が糖尿病性腎症による腎不全症例で, その5症例はいずれも起立性低血圧出現時にも心拍数の増加を認めなかった. 寒冷昇圧試験ではGroup 1で, 低反応を示す症例が3例に認められた. アドレナリン負荷試験ではGroup 1で全例が過大反応を示した....
