Se describe el caso clínico de una mujer que tuvo un papiloma invertido en seno esfenoidal.El esfenoides fue muy neumatizado y se extendió a receso lateral del seno, más allá la arteria carótida interna paraclival, procidente.Se hizo abordaje endonasal unilateral, combinando esfenoidotomía bilateral con septectomía posterior transpterigoideo, para exponer localizado detrás pared maxilar.El resecado forma completa, sin evidencia recurrencias.
