Abstract Hepatic hemangioma is the most common hepatic tumor with a prevalence of approximately 3%. It typically supplied by artery as evident from findings abdominal angiography, contrast‐enhanced ultrasonography (CEUS), computed tomography (CT), and magnetic resonance imaging. 肝臓,胆嚢,膵臓,脾臓,腎臓,腹部大動脈の6臓器を対象とした腹部超音波スクリーニング検査について解説をした.超音波検査は一度に表示できる範囲が狭いため何回かに分けてscanをすることが必要になる.この際,見逃しを防ぐ観点,第三者による読影が必要となる観点,新人教育の観点の3点から走査部位と撮影順番を決めて走査することを推奨している.保存画像については超音波検査の特徴を活かしたvolume data(1方向のsector scanの動画像を含む)が望ましい.しかし画像保存方法が各施設で統一されていないためその領域の観察を行った証として各臓器2方向から観察し,25枚の静止画像の保存を行うようにしている.ここでは各断面の観察のポイントおよび走査上の技術的なポイントを解説する.
肝における良性腫瘤性病変の超音波診断のポイントについて解説をする.良性腫瘤性病変における超音波診断の意義は,正確な診断を下し無用な二次検査を減らすことである.特に任意型検診で施行されることの多い超音波検査においては,過剰医療を防ぐためにも正確な超音波診断が重要となる.肝臓における良性腫瘤性病変の超音波診断においては,新たに特別な撮影を行うわけではなく,通常の超音波診断と同様に,まず肝臓全体の評価(びまん性肝疾患の有無),その後に腫瘤の存在診断(部位・腫瘍数・大きさ),次に質的診断を行う.質的診断にあたっては,腫瘍の形態,被膜の有無,境界領域(周辺・辺縁)の状態,内部構造の状態,非腫瘍部の状態,他の肝内病変との関係,などにより鑑別診断を行う.今回は,日常診療において比較的頻度の高い腫瘤性病変である,肝嚢胞,肝血管腫,限局性結節性過形成,肝細胞腺腫,血管筋脂肪腫,炎症性偽腫瘍,に対する超音波診断のポイントについて解説する.
