In the year 2000, organizational structure of ICU in Zaandam Medical Centre (ZMC) changed from an open to a closed format ICU. The objective this study was evaluate effect change on outcome high risk surgical patients. The medical records all consecutive patients admitted 1996 1998 (open format) and 2003 2005 (closed format), were reviewed. High-risk defined according Identification Risk Surgical (IRIS) score. Parameters studied were: mortality, morbidity, length stay (LOS) hospital...
Ventricular fibrillation is common in patients with out-of-hospital cardiac arrest. Early and effective defibrillation important for their survival. Effective depends highly on correct positioning of the pads. Teaching this correctly by ALS instructors therefore crucial. Fifty certified advanced life support were recruited from a large training institute. Participants asked to place pads an anatomically real-weight (90 kg) manikin. Primary outcome was placement placed sternal-apical...
Abstract Background The extent to which Point-of-care of ultrasound (POCUS) is used in different European helicopter EMS (HEMS) unknown. We aimed study the availability, perception, and future aspects POCUS HEMS using an online survey. Method A survey about use was conducted by a multinational steering expert committee carried out from November 30, 2020 December via web portal. Invitations for participation were sent email medical directors organizations including two reminding notes....
During the Coronavirus Disease 2019 (COVID-19) outbreak in Netherlands, demand for intensive care beds exceeded availability within days. Initially, patients were redistributed regionally by ground transport. When transport over longer distances became necessary, we initiated a new Helicopter Emergency Medical Service (HEMS) operation. We hypothesize that of contagious COVID-19 is feasible and safe HEMS personnel.
We studied the evolution of software in accuracy FloTrac/Vigileo system to measure cardiac output less invasively from arterial pressure waveform analysis without calibration, comparison with pulmonary artery catheter-derived thermodilution measurements, patients septic shock and presumed alterations vascular tone.Nine who received a catheter were on mechanical ventilation sinus rhythm monitored by FloTrac/Vigileo. Paired measurements both techniques analysed for 86 four using 1.07 version...
Cardiac output (CO) measurement is often required in critically ill patients. The performances of newer, less invasive techniques require evaluation patients with severe sepsis and septic shock. To compare calibrated arterial pressure waveform analysis-derived CO (COap, VolumeView/EV1000) the uncalibrated form (COfv, FloTrac/Vigileo) transpulmonary thermodilution derived (COtptd).A prospective, observational, single-centre study.ICU a general teaching hospital.Twenty consecutive or shock...
In Brief BACKGROUND: Hemodynamic variables can theoretically be influenced by a combined psoas compartment–sciatic nerve block (CPCSNB) owing to relatively high systemic absorption of local anesthetics and extended vasodilatation in the anesthetized limb (hemisympatectomy). this study we assessed documented hemodynamic changes during CPCSNB for elective orthopedic surgery. METHODS: Twenty consecutive patients scheduled total hip arthroplasty revision surgery were subjected with 150 mg...
Background In ambulatory lower limb surgery, spinal anesthesia with rapid onset and a short duration of block is preferable. We hypothesized that the use 2-chloroprocaine would be associated faster motor recovery compared prilocaine in knee arthroscopy. A difference 15 min was considered clinically relevant. Methods 150 patients were randomly allocated to receive intrathecally either 40 mg or prilocaine. The primary outcome time complete from blockade. Secondary outcomes included full...
Introducing advanced hemodynamic monitoring might be beneficial during Helicopter Emergency Medical Service (HEMS) care. However, it should not increase the on-scene-time, easy to use and non-invasive. The goal of this study was investigate feasibility non-invasive cardiac output measurements by electrical cardiometry (EC) quality EC signal pre-hospital care provided our HEMS.A convenience sample fifty patients who required HEMS assistance were included in study. Problems with respect...
Abstract Objectives The TensorTip™ MTX is a non-invasive device designed to determine several physiological parameters with additional analysis of haemoglobin, haematocrit and blood gas by interpreting diffusion colour the finger skin based on spectral analysis. aim our study was investigate accuracy precision TensorTip in clinical setting comparison routine samples. Methods Forty-six patients, scheduled for elective surgery, were enrolled this study. Placement an arterial catheter had be...
Cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) are high-risk extensive abdominal surgery. During surgery, less invasive methods for cardiac index (CI) measurement have been widely used in operating theater. We investigated the accuracy of CI derived from different (FroTrac, ProAQT, ClearSight, arterial pressure waveform analysis [APWA], PICCO) compared them to transpulmonary thermodilution (TPTD) during CRS HIPEC operative room intensive care unit (ICU).
Abstract Introduction A broad range of pathophysiologic conditions can lead to cardiopulmonary arrest in children. Some these children suffer from refractory cardiac arrest, not responding basic and advanced life support. Extracorporeal-Cardiopulmonary Resuscitation (E-CPR) might be a life-saving option for this group. Currently therapy is only performed in-hospital, often necessitating long transport times, thereby negatively impacting eligibility chances survival. We present the first two...
