To study ultrastructural changes in endocardial tissues and endocrine cardiomyocytes of the left atrial appendage patients with fibrillation.Electron microscopy was used to examine endocardium 8 long-standing paroxysmal persistent fibrillation one patient coronary heart disease without rhythm disturbance (a control group). The investigation revealed that all had similar layers appendage. The endothelium showed massive desquamation endothelial cells. Predominantly single sharply flattened...
Aim. The purpose of our study was to value the role potential arterial emboli sources in death cause according postmortem data collected deceased from all causes a multifield hospital.Material and methods. We performed retrospective analysis 1324 records died pa- tients for past 6 years (2011-2016). registered patients’ age sex, thrombus site, definite intravital clinical anamnestic data, individual features tanathogenesis.Results. Intravital formation found 64 patients (4,8%), mean 73,9±2,7...
Embologenic arterial obstruction remains an extremely important problem of modern medicine. Emboli may affect virtually all vessels the greater circulation and in some cases emboli be multiple. The purpose present study was to work out a classification multiple emboli. Analysing clinical material including over 30 years total 1804 patients with embolism aorta major arteries limbs made it possible define range terms used submit for discussion describing situation where simultaneously several...
Aim. To study the characteristics of interleukin-6 (IL-6), interleukin-1β(IL-1β), interferon-γ(INF-γ) secretion in rheumatic patients with and without anemia chronic diseases. assess influence these cytokines on development iseases various pathologies. propose a working version classification diseases based leading pathogenetic factor anemia. Material methods. 126 patients, 34 men (45,8 (36–54,9) years old), 92 women (49,5 (38–60) old) were examined. Group 1 included 41 ACD, iron deficiency...
Aim. To assess the potential of prevention cardiogenic embolism by resection left atrium appendage (LAA) during open heart surgery in patients with atrial fibrillation (AFib). Materials and methods. Study design - cohort prospective. group consisted 19 AFib whom for coronary and/or valvular disease additional radical LAA was made. After removal two-layer linear suture to performed without leaving a stump. Control 20 AFib, remained intact. Long-term results were studied using CROQ telephone...
The objective of the study was to reveal features development and clinical course acute arterial embolism (AAE) aorta main limb arteries in patients with myocardium infarction (MI). Material methods . Since 1991 until 2018 department cardiovascular surgery «North-Western State Medical University named after I. Mechnikov» 1782 AAE were urgently treated. 102 (5.7 %) them waswith IM. Results. In phase Q-IM large profile typical. led rapid progression ischemia. severity general condition due...
The OBJECTIVE of the study was to assess current trends in surgery acute upper limb ischemia (AULI). MATERIAL AND METHODS. We performed comparative analysis clinical data and treatment results 261 patients with AULI urgently treated between 2008 2018 years (main group) 162 1975 1985 (control group). RESULTS . For past 30 years, there were growing rate AULI. In 92.7 % cases, main cause embolism. disease leading embolism became atrial fibrillation 80.8 cases. decades, a significant improvement...
The Objective was to study our treatment experience for the past 50 years of patients with acute embolism aorta and main arteries limbs (EAMAL). Methods materials . We analyzed 3498 EAMAL over years, from 1971 2020. All were treated at a single department. It one first vascular surgery departments established in St. Petersburg. admitted clinic an emergency way, all them received urgent angiosurgical care. Results. 3091 (88.4 %) operated on, 2950 (95.4 cases primary operation...
Epidural administration of opioids effectively prevents the sensitization nociceptive neurons posterior horn spinal cord, induced by surgical trauma, without development motor and autonomic blockade. Study includes 52 patients aged 1 month to 18 years with malignant tumors who underwent treatment at Pediatric Oncology Hematology Research Institute, during 2009-2016 years. Surgeries were combined (thoraco-laparotomy). The method epidural use morphine in pediatric oncosurgery is presented.
Introduction. Acute limb ischemia due to embolism in patients with atrial fibrillation remains poorly studied. Objective – study the clinical significance and role of (AF) development bifurcation aorta main arteries limbs. Materials methods. Treatment results 1816 acute extremities treated at a specialized vascular surgical department for past 30 years were analyzed. 1611 (88.7 %) them had AF. The distribution into studied groups was according period time factor. Group 1 (n = 744) consisted...

Background: Fluid overload is an independent risk factor for acute kidney injury after cardiac surgery. An objective volume status assessment remains unsolved clinical problem. Objective: To analyze the relationship between dynamic changes in splanchnic blood flow patterns (portal, hepatic, and interlobar renal veins), reflecting venous congestion severity, severity of dysfunction studied patients. Methods: From February to May 2023, 3 patients open heart surgery developed postoperative...
BACKGROUND: In patients with atrial fibrillation, systemic thromboembolic complications dramatically worsen the long-term prognosis. There is currently no generally accepted treatment tactics for fibrillation of non-valvular ethiology after embolism to main arteries limbs. Objective: evaluate efficacy our approach fibrillationof who survived an limbs and acute limb ischemia.
MATERIALS AND METHODS: For period from 1991 2022, in Department Vascular Surgery institution, emergency care due...
Different issues regarding the arrangement of urgent medical care to patients with embolic events aorta and main arteries limbs in Saint-Petersburg for past 25 years were studied. There was performed a comparative analysis outcomes 863 patientswith acute limb ischemia treated specialized vascular surgery department 897 ingeneral departments. The superiority surgical revascularization under conditions demonstrated.
We report herein, successful thoracoscopic management of a patient who had foreign body in the superior vena cava. Foreign cava this was accompanied by thrombosis, thus limiting use endovascular extraction. The impacted bode successfully removed minimally invasive approach without any major or minor traumas. This clinical case proved that removal is safe alternative group patients. Given low prevalence type complications and excellent postoperative outcome, should be considered as one...
