A rare case of extradural brucellosis granuloma in the thoracic region is presented. In spite medical treatment, total paraplegia with sphincter disturbance developed. Delayed neurosurgical intervention resulted significant clinical recovery. Magnetic resonance imaging was useful obtaining detailed information and to decide on surgery.
