耳下腺癌は症例数が少なく,また悪性度の低い症例もあるため長期間の観察が必要であり治療成績を検討することが容易でない。1999年9月から2019年2月までの約19年間に当科で加療を施行した耳下腺癌新鮮症例は184例であった。ステージ別ではステージⅠが22例,Ⅱが71例,Ⅲが23例,Ⅳが68例であった。悪性度別では低・中悪性が105例,高悪性が79例であった。主な組織型は粘表皮癌が47例,多形腺腫由来癌が27例,腺様囊胞癌が21例,唾液導管癌が15例,分泌癌が12例,腺房細胞癌が11例であった。症例全体の疾患特異的5年生存率は76.6%であった。ステージ別の疾患特異的5年生存率はステージⅠが100%,Ⅱが97.7%,Ⅲが71.6%,Ⅳが51.6%であった。悪性度別の疾患特異的5年生存率は,低/中悪性が95.9%,高悪性が46.1%であった。高悪性癌は最も大きな予後不良因子であるが,術前の悪性度診断は必ずしも良好ではなかった。
