ABSTRACT New‐onset lupus nephritis (LN) poses a diagnostic and management challenge during pregnancy. We present the case of a postpartum woman presenting with acute dyspnoea and hypoxia. She was found to have liver enzyme derangement evidence synthetic dysfunction. Computed tomography her chest showed ground-glass opacities. The causes this phenomenon are discussed, including difficulties in differentiating amniotic fluid embolism fatty pregnancy. Pulmonary fat is also considered as differential diagnosis.
