本文报道1例发音纽安装术后无法重新发音,应用食道测压定位联合超声引导下肉毒素注射治疗后发音改善的病例。患者男,65岁,因“全喉切除术后Ⅱ期发音纽安装术后1年余,无法发声”就诊。考虑为发音时咽食管括约肌痉挛所致。采用食道测压定位,测得传感器距前鼻孔20 cm处,发音时存在一高压区压力均值为(73.6±10.7)mmHg(1 mmHg=0.133 kPa),此处即为痉挛部位,于该部位注射1%利多卡因5 ml后,患者可间断发音“a”,发音时压力显著下降。于同一部位,超声引下多点注射10 U/ml肉毒素A 50 U。术后3 d患者可连续发音10 s以上,复测发音时压力较术前显著下降。术后2周,患者可正常交流。术后半年,患者仍可正常交流。.
