The PEGASUS-TIMI 54 (Prevention of Cardiovascular Events in Patients with Prior Heart Attack Using Ticagrelor Compared to Placebo on a Background Aspirin–Thrombolysis In Myocardial Infarction 54) trial studied 2 doses ticagrelor, 90 mg twice day (bid) and 60 bid, for long-term prevention ischemic events patients prior myocardial infarction. Both similarly reduced the rate versus placebo. pharmacokinetics pharmacodynamics ticagrelor bid have not been studied. this study, authors sought study...

Summary Diabetes increases cardiovascular risk and reduces pharmacodynamic response to some oral antiplatelet drugs. This study aimed determine whether ticagrelor 60 mg twice daily (bid) provided potent consistent platelet inhibition in patients with vs without diabetes the PEGASUS-TIMI 54 function substudy. Out of 180 studied, 58 were randomised had received at least four weeks bid, 20 (34 %) having diabetes, 90 12 (21 64 placebo, 18 (28 diabetes. Blood was sampled pre- 2 hours...

A new system is described for catheter ablation which comprises a short time capacifive power source and specially designed catheter. The aim of the to deliver high voltages with minimal or no arcing thus avoid risks associated barotrauma. performance was compared in saline tank that conventional system. allowed significant increases delivered voJtage, current energy without arcing. This shouJd allow safer ablation.

A power source modified to increase voltage delivery and minimise arcing (for a given energy) was used for transvenous ablation of atrioventricular conduction control refractory supraventricular arrhythmias in 14 patients. Twelve had atrial fibrillation or flutter, one nodal reentry tachycardia, the other permanent junctional tachycardia. Despite treatment with 5-7 (median 6) antiarrhythmic drugs symptoms persisted all Cathodal discharges 0.5-39.5 J were delivered distal electrode (in case...

A 23‐year‐old man was resuscitated from ventricular fibrillation and subsequently shown to have the Wolff‐Parkinson‐White syndrome. Electrophyslological study demonstrated a posteroseptal accessory pathway, coronary sinus angiography that this associated with diverticulum of sinus. Catheter ablation performed using new low energy system. Five shocks were delivered within diverticulum, cumulative 39 joules (J). Accessory pathway conduction blocked successfully, there no complications.

Middle-Income Trap (MIT) is a phenomenon wherein nation finds itself unable to progress from middle-income status high-income economy. Despite Bangladesh’s rapid economic growth as developing economy following its transition the lower-middle-income category, country faces various challenges that may impede advancement higher-income tiers. Consequently, this study delves into whether Bangladesh can successfully navigate way out of and upper-middle-income traps. To assess this, initially...

To our knowledge we report the first case of percutaneous mechanical thrombectomy used for treatment massive pulmonary embolism in United Kingdom. Pulmonary is a common disease process but can be difficult to diagnose. Massive presenting with profound hypotension, however, rare. Both phenomena carry them significant mortality. Traditionally those patients suffering haemodynamic compromise from are treated intravenous or catheter-directed thrombolysis. When this contraindicated surgical...

She was intolerant of antiarrhythmic medication underwent low energy catheter ablation. Six non-arcing shocks 25 J were delivered to the tract. No further tachycardia occurred during follow up seven months without treatment.

This study has used high energy ablative shocks in saline and man to characterize the complex, dependent behavior of impedance at electrode-tissue interface. In vitro showed a fall when an arc was formed, due formation around electrode vapor globe rendered conductive by arc. Following extinction, this same insulates electrode, subsequently collapses, forming shock wave, allowing current flow resume. Shocks delivered tissue specimens exhibited progressively higher less insulation as contact...
