Malignant progression and survival analysis for intraductal papillary mucinous neoplasms based on risk stratification

ABSTRACT
Objective To evaluate the potential malignancy, prognosis and risk factors for intraductal papillary mucinous neoplasm (IPMN), which were classified into different levels based on Fukuoka guideline. Methods A retrospective analysis of patients with IPMN diagnosed at Nanjing Drum Tower Hospital from 2009 to 2016 was conducted. Clinical characteristics, treatment IPMNs analyzed. Results A total 94 included divided 3 groups according guideline, 46 in high-risk(HR) group, 30 group worrisome features (WF), 18 low-risk(LR) group. For undergoing surgery treatment, there 5 cases (19.2%, 5/26) HR 2 (12.5%, 2/16) WF whose postoperative pathological findings malignant (P=0.690). The 5-year survival rates after operations 73.9% 77.0% respectively (P=0.830). without a follow-up, 6 (33.3%, 6/18), (16.7%, 2/12) 0 (0.0%, 0/18) progressing pancreatic cancers HR, LR groups, (P<0.05). In addition, among three 49.5%, 85.7% 100.0% (P=0.025). Jaundice significantly related (P<0.01) hazard ratio 8.883 (95%CI: 2.953-26.721). Conclusion Jaundice is predictive factor IPMN. As IPMN, should receive while those can be followed up. customized, evaluation factors, performed when needed. Key words: Prognosis; Jaundice; Intraductal neoplasm; Risk stratification; Malignancy
