【背景】救命救急センター等で加療が必要な患児を検出する目的で,東京都では病院前救護における乳幼児緊急度判断項目が策定され2009年10月から運用を開始している。【目的】乳幼児緊急度判断項目の妥当性の検証。【対象】2011年2月から2012年3月までの14か月間に,国立成育医療研究センターへ救急車で直接搬送された16歳未満の症例。【方法】診療録を後方視的に検討した。外来死亡・蘇生処置施行・小児集中治療室入室のいずれかに該当するものを緊急度・重症度が「高い」,それ以外は「低い」とした。乳幼児緊急項目に該当した症例のうち緊急度・重症度が高いと判定されたものを適中例,逆に緊急度・重症度が低いと判断されたものをオーバートリアージ(OT)例,さらに乳幼児緊急項目に該当せず緊急度・重症度が高いと判定されたものをアンダートリアージ(UT)例とした。【結果】適中例は41/2,707例。内因性症例が大半を占め,疾患としては中枢神経系の異常が多く,陽性基準項目としては意識レベルの異常への該当が多かった。OT例は69/2,707例。うち75%が熱性痙攣症例であった。一方,UT例は62/2,707例で,外...
