A case of primary anaplastic pilocytic astrocytoma (PA) and its clinicopathologic radiologic features are reported. PA is a World Health Organization grade I tumor typically affecting children young adults. Initial presentation with particularly uncommon, making definitive diagnosis can be challenging.
