Recent randomized controlled trials comparing neoadjuvant chemoradiation plus surgery or perioperative chemotherapy with alone showed significant survival benefits for combined modality treatment of patients localized esophageal adenocarcinoma. However, head-to-head comparisons and applying contemporary protocols are lacking. The present trial was initiated to obtain valid information whether yields better in the adenocarcinoma. The ESOPEC is an investigator-initiated multicenter prospective...

Polyethylene glycol (PEG)-based gut lavage solutions are safe and effective, but require consumption of large volumes fluid. We compared a new 2 L solution PEG plus ascorbic acid (PEG + Asc) with standard 4 electrolytes E) for bowel cleansing before colonoscopy to determine efficacy, safety, patient acceptability.Consenting adult inpatients scheduled undergo were randomized receive either Asc or E. Preparations taken as split doses the evening following morning. The group took 1 at each...

OBJECTIVES: Endoscopic removal of large, nonpedunculated colorectal lesions is challenging. Long-term outcome data based on standardized protocols, including detailed inspection the resection site, are scarce. The aims present study were to evaluate safety and efficacy endoscopic (ER) (LNLs; >20 mm) assess long-term recurrence rate afterward. METHODS: A total 243 consecutive patients (141 men, 102 women) with 252 adenomas (>20 was followed up using a protocol after complete ER. After...

Objective
Although it is well understood that the risk of oesophageal adenocarcinoma increases with Barrett length, transition risks for cancer associated different lengths are unknown. We aimed to estimate annual rates patients long-segment (≥3 cm), short-segment (≥1 <3 cm) and ultra-short-segment (<1 Barrett9s oesophagus.
Design
used three data sources each length category: (1) distribution long, short ultra-short oesophagus among a large German cohort newly...

Background and study aim: It is commonly assumed that ablation of any remaining Barrett’s epithelium after endoscopic resection early neoplasia improves outcome by reducing the rate metachronous lesions, but this has not yet been evaluated in a randomized trial. The aim was to compare argon plasma coagulation (APC) with surveillance only for management residual following resection. Patients methods: Patients whom focal (high grade intraepithelial [HGIN] or mucosal cancer) had...

Sedation has been established for GI endoscopic procedures in most countries, but it is also associated with an added risk of complications. Reported complication rates are variable due to different study methodologies and often limited sample size.Acute sedation-associated complications were prospectively recorded electronic endoscopy documentation 39 centres between December 2011 August 2014 (median inclusion period 24 months). The sedation regimen was decided by each centre.A total 368...

In recent years, short-segment Barrett's esophagus (SSBE) has attracted increasing attention in the context of reflux disease. However, there is continuing controversy regarding its potential for malignant transformation.Between October 1996 and September 1999, 50/115 patients (43 %) with intraepithelial high-grade neoplasia or early adenocarcinoma, who underwent local endoscopic treatment, had developed a lesion an (SSBE). framework prospective observational study, 28 were treated mucosal...

Endoscopic resection has been recommended as a local curative approach for Barrett's neoplasia, but large series are still rare. In the present study we analyzed histological characteristics of endoscopic specimens neoplasia.742 obtained from 326 patients were assessed. The following evaluated: type grade differentiation, depth infiltration, invasion into lymphatic and blood vessels, status (tumor-free margins regarded indicating R0 status).31 had no neoplasia excluded analysis. Background and Study Aims: There are conflicting data regarding the optimal bowel preparation for colonoscopy. This study was carried out to compare efficacy, safety, tolerability of three widely used lavage solutions: standard polyethylene glycol-electrolyte solution based on GoLytely formulation (PEG-EL1; Klean-Prep); a sulphate-free PEG-EL NuLytely (PEG-EL2, Endofalk); sodium phosphate (NaP, Fleet Phospho-Soda). Endoscopic ultrasound (EUS) has been regarded as the most accurate staging tool in esophageal cancer. Staging results have a strong impact on decision to whether patient should undergo endoscopic treatment, surgery alone, or neoadjuvant therapy. This retrospective study was conducted analyze accuracy of cancer using EUS.All patients who received EUS for before esophagectomy from February 2003 December 2007 at high volume academic tertiary care center were included.179 consecutive (mean age...

BACKGROUND Capsule endoscopy (CE) is a new modality for obscure digestive bleeding (OBD), but little known about its influence on management and outcome. PATIENTS AND METHODS Fifty-six patients (male/female 26/30; mean age 63 yr) with ODB, negative upper lower gastrointestinal (GI) were included in this multicenter study. The diagnostic yield of CE was compared to three other tests (OT: push enteroscopy, enteroclysis, angiography), followed up at least 6 months. Parameters analyzed that led...

Both were successfully sealed one clip. While patient recovered without reintervention, in other leak reappeared displacement 13 days later.

Thermal ablation for Barrett's oesophagus has widely been established in gastrointestinal endoscopy during the last decade.
