Puffer fish (Lagocephalus sceleratus) is a well-known source of deadly food poisoning due to neurotoxin, called tetrodotoxin. Cases tetrodotoxin are common along the coasts East Asian countries but rare in Arabian Gulf region. Here, we report case 19-year-old man who presented with signs and symptoms suggestive puffer poisoning. Dietary history was key diagnosis though laboratory investigations imaging yielded normal results. Early proper supportive management essential for survival.
