Neuron-specific enolase (NSE) is one of the biomarkers used as an indicator brain disorder, but since it also found in blood cell components, there a concern that spurious increase NSE may occur after cardiovascular surgery, where cardiopulmonary bypass (CPB) causes hemolysis. In present study, we investigated relationship between degree hemolysis and surgery usefulness immediate postoperative values diagnosis disorder. A retrospective study 198 patients who underwent with CPB period from...
Abstract Background This study explored if long-distance transfer was safe for patients suffering from acute aortic dissection type A (AADA) and also analyzed the effectiveness of helicopter cloud-type imaging systems such in northern Hokkaido, Japan. Methods Results The included 112 consecutive who underwent emergency surgical treatment AADA April 2014 to September 2020. were divided into two groups according location referral source hospitals: Asahikawa city group (group A, n = 49)...
This study aimed to compare the short- and long-term outcomes of proximal repair vs. extensive arch surgery for acute DeBakey type I aortic dissection. From April 2014 September 2020, 121 consecutive patients with A dissection were surgically treated at our institute. Of these patients, 92 had dissections extending beyond ascending aorta.Of 58 underwent repair, including root and/or hemiarch replacement, 34 extended partial total replacement. Perioperative variables early late postoperative...
To evaluate the effectiveness of 5-minute test (FMT), developed to record amount pericardial bleeding in patients undergoing general cardiac surgery, and determine relationship between this postcardiotomy bleeding. The medical records 573 who underwent adult surgery January 2016 December 2019 were reviewed retrospectively. Patients divided into 2 groups: FMT group included 2018 (n = 278), control 2017 295). The reexploration rate due intrapericardial or tamponade within 1 week after until...

Abstract We report the case of a 76-year-old woman with an incomplete atrioventricular septal defect and severe congestive heart failure who underwent surgical repair. Surgical intervention involved mitral valve repair patch closure ostium primum defect, resulting in favorable postoperative course. Successful outcomes support surgery as reasonable treatment option owing to its significant improvement quality life, even elderly patients left degeneration.
Abstract Nonbacterial thrombotic endocarditis (NBTE) on the aortic valve involves fibrin and platelet aggregate formation, potentially leading to embolic events. We present a case of NBTE following coronary angiography (CAG) in 54-year-old man with multiple comorbidities. Surgical thrombectomy was performed owing acute cerebral infarcts. This highlights significance considering that mechanical trauma from catheterization during CAG can trigger thrombus formation.
Abstract This case report details the management of a 79-year-old man who developed massive postoperative pneumothorax following redo coronary artery bypass grafting due to severe lung adhesions. We successfully treated patient using veno-venous extracorporeal membrane oxygenation without femoral cannulation, allowing for early rehabilitation initiation. Veno-venous is reasonable option cases respiratory failure with destruction caused by re-sternotomy during re-do cardiac surgery.
The proper surgical strategy for retrograde type A aortic dissection (RTAD) is still controversial, and some studies have reported the efficacy of frozen elephant trunk thoracic endovascular repair (TEVAR). 68-year-old man was diagnosed with acute using enhanced computed tomography. false lumen at arch ascending aorta thrombosed, primary entry placed in descending aorta. In addition, there were malperfusions right renal artery both iliac arteries. We performed TEVAR femoral combined...
Secure proximal anastomosis is an essential part of surgical treatment for acute aortic dissection type A (AADA). This study aimed to investigate the effectiveness modified turn-up technique in AADA and compare this with other techniques. We divided 57 patients who underwent ascending aorta replacement into group (group A: 36 patients) B: 21 patients). Intraoperative postoperative course data were compared between groups B. In A, we also early-career surgeons (practicing <10 years after...
Triplex (Terumo Corp, Tokyo, Japan) is a relatively new vascular protheses with non-biodegradable coating material. We experienced two cases of graft elongation in grafts post-operatively. In one the cases, led to occlusion left subclavian artery. other case, resulted pseudoaneurysm ascending aorta. A unique feature that they may reduce post-operative inflammation reaction; however, could also invite limited adhesion formation surrounding tissue, which contribute prostheses elongation, due...
Abstract If multiple treatments are performed within a short time, when something occurs, it is difficult to identify its cause. Here, we present case of thoracic endovascular aortic repair (TEVAR) for acute dissection (AAD) after treatments. A 76-year-old woman underwent minimally invasive valve replacement, transcatheter lumbar artery embolism and retroperitoneal tumor resection period time. After series procedures, the patient experienced sudden back pain, computed tomography revealed an...
For patients with cardiogenic shock, delaying surgery mechanical circulatory support is reported to yield better outcomes than emergency surgery. We report on an 82-year-old man diagnosed vertebral osteomyelitis concomitant infective endocarditis. Chest radiographs revealed a growing abscess, which resulted in aorto-right ventricular fistula. Providing Impella allowed for hemodynamic stabilization prior The patient had uneventful postoperative course and be well follow-up 1 year later. can...
Minimally invasive direct coronary artery bypass is preferred due to its less nature; however, it carries the risk of graft failure owing inherent technical challenges. We present a case where minimally grafting was performed and detected via angiography 3 days post-operation. Successful percutaneous intervention subsequently on failed itself salvage myocardial cellular damage. Consequently, combination intervention, both less-invasive revascularization approaches, effectively achieved...
Cases that are inoperable owing to poor preoperative conditions sometimes encountered. However, there some cases led radical treatment by performing bridge therapy. Here, we presented a case of patient with complex cardiac disease in an state who underwent bridging therapy successful surgical treatment. A 73-year-old male received hemodialysis and had severe aortic valve stenosis coronary artery planned he was deemed his low function hemodynamic instability. Therefore, escape from fatal...
Acute coronary syndrome with cardiogenic shock is a life-threatening condition, but planned staged treatment combined revascularization and mechanical circulatory supports its management increasingly possible. Here, we present our successful life-saving case. A 76-year-old male patient was diagnosed ST-elevation myocardial infarction due to severe stenosis of the left main artery based on triple vessel disease. We initially introduced Impella CP performed percutaneous intervention without...
The vibration of human-mechanical coupled system is investigated taking the unconscious muscle operation human body into consideration. The motion constrained in vertical two-dimension. characteristic this discussed by comparing with mechanical equivalent human-body-mass. Moreover, influences feedback parameters concerning on are clarified.
Central MessageIn CABG for HD patients, an in situ ITA graft ipsilateral to the upper-extremity AVF as a sole inflow should be avoided prevent aggravation of hemodynamics induced by coronary steal. In artery bypass grafting patients requiring chronic hemodialysis, there is possibility steal associated with using internal thoracic (ITA) arteriovenous fistula (AVF). Here, we present case left inflow. Immediately after surgery, patient experienced shock cardiopulmonary resuscitation. The data...
The extraction of a pacemaker (PM) lead may cause tricuspid regurgitation; however, in cases device infection, surgical intervention or immediate PM system replacement is undesirable the short term to prevent reinfection. We describe case where Micra leadless was used as bridge procedure ensure an adequate period for antibacterial therapy and later replaced with permanent setting pocket infection.
Abstract PurposePostoperative atrial fibrillation is a poor prognostic factor that increases mortality rates. We have observed patients who experienced significant pericardial effusion developed postoperative fibrillation; however, little research has been conducted examining the association between and post-cardiotomy effusion.MethodsThis retrospective, single-center study included adult underwent cardiovascular surgery via median sternotomy from January 2016 to December 2019. Patients...
