患者女,51岁,12年前胸部外伤后剧烈疼痛,被当地医院诊断为胸腔积液,此后每年行X线胸透检查示胸腔积液无明显变化.2年前开始胸闷,未治疗.2个月前出现咳嗽、咳灰黄色黏痰,痰中带血,量不多,初为暗红色血块,后转为鲜红色血丝.X线胸透示胸腔积液增多.患者始终无发热、盗汗及乏力等症状.查体:右侧胸廓饱满,呼吸活动度减低,叩音实,呼吸音极低。
