症例は千葉県在住の31歳,女性.腹痛と下痢のために近医を受診.腹部造影CT検査で肝内に多発する嚢胞性病変を認めた.エキノコックス抗体検査が陽性で肝多包性エキノコックス症の診断となり,手術目的に当科紹介となった.嚢胞は肝S3/2,S5,S7/8,S8に存在し,S8の嚢胞は右肝静脈に近接する病変で,嚢胞壁の損傷を回避するため右肝静脈を含む拡大後区域切除術,外側区域切除術,S5部分切除術を行った.術後経過は良好で術後第18病日に退院した.病理組織所見では類上皮肉芽腫と嚢胞内部に大型の壊死形成を認めた.壊死部にはクチクラ層を有する小嚢胞を認め,繁殖胞を有する原頭節も含まれていた.術後3年経過したが,再発なく生存中である.本症例は関東在住であるが,2歳時に北海道へ旅行し,キツネとの接触歴があった.感染後29年の経過で緩徐に進行したと考えられる肝多包性エキノコックス症を経験したので報告する.
