大型髄膜腫により上矢状静脈洞の狭窄・閉塞を認める症例では板間静脈が側副血行路として発達していることがある. 大出血や静脈性梗塞を防ぐためにはこれら板間静脈を可能なかぎり温存することが望ましいが, 板間静脈の温存法について言及している文献は乏しい. 今回, われわれは板間静脈を温存したうえでの腫瘍切除を行ったため報告する. 3DCTAは術前に腫瘍, 上矢状静脈洞および板間静脈の位置関係の把握に, 脳血管造影検査は詳細な静脈灌流動態の把握に有用であった. 術中はこれらの情報をもとに板間静脈を損傷しない開頭が必要であった.
