Minimally invasive cholecystectomy and common bile duct exploration offers single- stage management for complex gallstone disease (cholelithiasis choledocholithiasis). Robotic platform presents benefits in improving operative precision, which has not been extensively evaluated the acute setting of managing disease, as well performing transcholedochal or post exploration. We report an early series emergent expedited robotic assisted with (RC-CBDE) (R-CBDE) alone.

Inguinal hernias are a common presentation to surgical admission units throughout the world. The majority of presentations due containing either fat or small bowel. However, wide range intra-abdominal viscera have been demonstrated in inguinal hernias. We report case an 87-year-old man who presented with gastric outlet obstruction secondary incarcerated hernia pylorus.

The primary objectives include examining how AI tools impact user behavior measuring their effects engagement metrics. A quantitative analysis 350 participants revealed a significant positive correlation between filter usage engagement, with coefficients 0.65 for likes 0.58 comments. Complications of laparoscopic adjustable gastric bands (LAGB) are well documented and may include migration, erosion, slippage, infection, pouch dilatation and, rarely, perforation. We describe a rare case involving three such complications simultaneously, namely, infection migration through the pylorus-causing proximal jejunal obstruction. As LAGB is now commonest performed bariatric procedure for treatment morbid obesity, we encourage practising surgeon to be vigilant these but potentially...

Introduction: Intervention options in acute cholecystitis (AC) include drainage (percutaneous/endoscopic) or surgery. Several scoring systems have been used to risk stratify surgical patients, but few validated. This study investigated the suitability of Acute Physiology and Chronic Health Evaluation II (APACHE-II) score, American Society Anesthesiologist (ASA) grade, Tokyo Guidelines 2018 (TG18) grade as predictors outcome assess laparoscopic cholecystectomy versus percutaneous...

Abstract Background A recent large retrospective cohort study reported a higher risk of bile duct injury associated with robotic cholecystectomy when compared to the laparoscopic approach. Considering that most training pathways, including one followed in UK, utilise as index procedure, such findings mandate evaluation safety profile UK ensure its use an alternative approach is not putting patients at risk. We aimed conduct multi-centre observational evaluate performed within HPB Robotic...

Abstract Introduction Revisional bariatric procedures are increasingly prevalent. These more complex with higher risk of complications as compared to primary procedures. The potential indications for revisional laparoscopic gastric bypass (R-LRYGB) may include weight regain and gastroesophageal reflux disease (GORD). This video abstract presents role fundectomy Indocyanine green fluorescence angiography (ICG-FA) perfusion assessment pouch in R-LRYGB. Methods Operative technique A 40-year-old...

Abstract Introduction Indocyanine green (ICG) fluorescence angiography (FA) has been utilised in bariatric surgery for assessment of tissue perfusion and blood supply laparoscopic sleeve gastrectomy (LSG). However, there is limited literature on its use Roux-en-Y gastric bypass (LRYGB). The aim this study was to evaluate the ICG-FA LRYGB qualitative structure identification. These are first cases utilising be performed United Kingdom. Methods Thirty-five patients (12 male, 34%, 23 females,...

Mr Awan and his colleagues (July 2004 JRSM1) report a case of malrotation in an adult. As they point out, this condition is much more often encountered childhood. Paediatric surgeons would not agree that the operation done case, with attempt to restore normal anatomy, was correct one. The standard operative procedure based on described by Ladd 70 years ago—detorsion volvulus, division adhesions, broadening small-bowel mesentery positioning duodenum down right side abdomen caecum into left...

Abstract Aim Acute small bowel obstruction secondary to strangulated obturator hernia is a rare condition, with high rates of morbidity and mortality in the absence prompt diagnosis intervention. We aim describe case above presentation, managed using minimally-invasive approach positive outcomes. Material Methods an 82-year-old female who presented acute on cross-sectional imaging.
