【目的】比較的稀とされるAnterior condylar confluent dural AVF(ACC DAVF)に対して当院で治療経験した症例群のその臨床的特徴,治療方法,転帰をまとめる.【方法・結果】2011 年4 月から2014 年8 月に至るまで,当院で治療したACC DAVF は5 例6 病変で,平均年齢65.6 ± 8.9 歳,全例男性であった.病片側は,左2 例,右2 例,両側1 例であった.いずれも耳鳴を主訴に発見された.流入動脈は,上行咽頭動脈5 例,後頭動脈2 例,椎骨動脈4 例であった.4 例でシャント部にvenous pouch の形成が確認でき,その大きさは平均9.8 2.5 mm であった.静脈に逆流する血行動態を持つものは6 例中4 例であった.全例で,経静脈的にtargeting embolization が可能で,うち3 例でmicro balloon を併用した.その結果,使用したコイルは平均10.4 2.87 個,コイル長は平均100 37.6 cm...

ヘパリンは,血管内治療において一般的に使用されているが,術中のヘパリン起因性血小板減少症に関しては,あまり知られていない.患者は52歳女性.頭痛後に意識障害を呈し,救急搬送された.CTによりSAHと診断し,MRAで脳底動脈–左上小脳動脈分岐部に動脈瘤を認め,コイル塞栓を行った.治療中にヘパリン起因性血小板減少症によると考えられた多発血栓を認め,直ちにアルガトロバンによる治療を開始したが,多発脳梗塞を来した.脳外科領域の血管内治療中にヘパリン起因性血小板減少症を呈した報告は少なく,破裂脳動脈瘤におけるコイル塞栓中の発症は過去1例であった.ヘパリン起因性血小板減少症は稀であるが,重篤な合併症を来し得るため,早期に発見し,治療を開始することが重要である.
