Two new monomeric and one dimeric carbazole alkaloids were isolated from root bark of Murraya euchrestifolia HAYATA collected in Taiwan. Their structures elucidated by spectrometric synthetic studies. The the carbazoles assigned as 3-formyl-7-hydroxy-9H-carbazole (1) N-methoxy-3-hydroxymethyl-9H-carbazole (2). carbazole, named chrstifoline-D (9), was found to be identical with oxidation product bismurrayafoline-A (10).

大阪成人病センターで1981年までに手術した34歳以下の胃癌113例中, 男性は50例 (男性全体の2.5%), 女性は63例 (7.4%) で女性は非常に高率であった (p<0.05). 男性は早期癌 (男性中46%, 女性中26%, p<0.05) が高率で, 女性はstage IV進行癌 (男性中16%, 女性中33%, が高率であった. 女性の分娩歴を調査したところ, 分娩から手術までの期間が短いほどstage IVが高率で (分娩後2年未満38%, 2年~6年未満40%, 6年以上29%), 妊娠経験のないものは男性とほぼ同じ率を示した (13%). 妊娠, 分娩は若年女性胃癌の進行に影響を与えていると考えられた.
手術可能な進行皮膚悪性腫瘍の遠隔成績は必ずしも良くなく,補助的治療法が模索されているが,根治術後化学療法の意義は未だ明らかになっていない。今回我々は,皮膚悪性腫瘍(有棘細胞癌,皮膚附属器癌,乳房外Paget病)の局所進行例,リンパ節転移症例5例を対象に,5-FU系経口抗悪性腫瘍薬S-1を用いた術後補助化学療法のpilot feasibility studyを行った。S-1 80mg/m2/日,2週投与2週休薬で1年間内服を目標とした。副作用は軽度で,再発のため中止した症例を除き5例中4例で完遂できた。S-1は服薬コンプライアンスも良好で,安全に施行できる方法と考えられた。本法の有効性はいまだ不明であり,症例集積,比較臨床試験が必要と考えられるが,皮膚悪性腫瘍に対する根治術後補助化学療法として検討される価値があるかもしれない。
45歳, 男性。1年前に左頬部に腫瘤が出現した。前医で生検を施行されmyxofibrosar-comaが疑われた。当科初診時, 4.5×3.5cm大の弾性軟な腫瘤を認めた。治療は2cmのwide excisionになるように頬骨骨膜と咬筋の一部を切除した。切除後組織欠損はcervicofacial flapで再建した。病理組織学的検査では, 真皮から筋層にかけて多量のムチンを含む比較的境界明瞭な結節性病変があり, 軽度の異型性のある円形や紡錘形の核をもつ細胞が増加していた。免疫組織化学染色ではα-SMA, desmin, S-100蛋白, cytokeratinは陰性で, CD68は陽性であり, myxofibrosarcoma, FNCLCCによる組織学的悪性度はGrade 1, AJCC systemではStage IAと診断した。 補助療法は行わず, 術後2年を経過して再発, 転移は認めていない。
Rosai-dorfman病(Rosai-Dorfman Disease;RDD)は,原因不明の組織球増殖性疾患である。主病変はリンパ節であるが,節外病変を認めることもあり,皮膚に病変が限局する症例は皮膚Rosai-dorfman病(cutaneous Rosai-Dorfman Disease;cRDD)として報告されている。
Angioleiomyomaは皮膚平滑筋腫の一型で,血管壁の平滑筋に由来する良性腫瘍である。多くは中高年女性の下肢に好発するが,耳介に発生した報告は極めて少ない。今回我々は,耳輪に発生したangioleiomyomaの症例を経験したので,若干の文献的考察を加え報告する。症例は21歳,男性。初診の数年前より徐々に増大を自覚し,近医でMRI検査を施行したところ脂肪腫が疑われたため当院を紹介され受診し,パンチ生検を施行したが確定診断に至らなかったため,切除術が計画された。局所麻酔下にて完全切除し,切除検体の病理組織学的所見でangioleiomyomaと診断した。現在,術後5ヵ月を経過するが予後は良好にて局所再発はみられていない。
