We report the case of a kidney transplant recipient with invasive aspergillosis due to Aspergillus fumigatus resistant voriconazole and intermediately susceptible posaconazole who failed therapy. Plasma concentrations indicated an unfavorable ratio area under concentration-time curve over MIC.
