Nasal carriers of Staphylococcus aureus are at increased risk for health care-associated infections with this organism. Decolonization nasal and extranasal sites on hospital admission may reduce risk.In a randomized, double-blind, placebo-controlled, multicenter trial, we assessed whether rapid identification S. by means real-time polymerase-chain-reaction (PCR) assay, followed treatment mupirocin ointment chlorhexidine soap, reduces the hospital-associated infection. From October 2005...

Abstract In 2003 in the Netherlands, a new methicillin-resistant Staphylococcus aureus (MRSA) strain emerged that could not be typed with Sma1 pulsed-field gel electrophoresis (NT-MRSA). The association of NT-MRSA humans reservoir animals was investigated. frequency increased from 0% 2002 to >21% after intensified surveillance implemented July 2006. Geographically, clustered pig farming. A case–control study showed carriers were more often or cattle farmers (pig odds ratio [OR] 12.2, 95%...

The choice of empirical antibiotic treatment for patients with clinically suspected community-acquired pneumonia (CAP) who are admitted to non-intensive care unit (ICU) hospital wards is complicated by the limited availability evidence. We compared strategies (allowing deviations medical reasons) beta-lactam monotherapy, beta-lactam-macrolide combination therapy, or fluoroquinolone monotherapy. In a cluster-randomized, crossover trial rotated in 4-month periods, we tested noninferiority...

Importance
On February 27, 2020, the first patient with coronavirus disease 2019 (COVID-19) was reported in Netherlands. During following weeks, at 2 Dutch teaching hospitals, 9 health care workers (HCWs) received a diagnosis of COVID-19, 8 whom had no history travel to China or northern Italy, raising question whether undetected community circulation occurring.
Objective
To determine prevalence and clinical presentation COVID-19 among HCWs self-reported fever respiratory...

10 days after the first reported case of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection in Netherlands (on Feb 27, 2020), 55 (4%) 1497 health-care workers nine hospitals located south had tested positive for SARS-CoV-2 RNA. We aimed to gain insight possible sources workers. We did a cross-sectional study at three Netherlands. screened participating infection, based on clinical symptoms (fever or mild symptoms) before screening. obtained epidemiological data through...

Background. The worldwide prevalence of extended-spectrum β-lactamase (ESBL)-producing Enterobacteriaceae is increasing rapidly both in hospitals and the community.A connection between ESBL-producing bacteria food animals, retail meat, humans has been suggested. We previously reported on genetic composition a collection Escherichia coli (ESBL-EC) from chicken meat restricted geographic area. Now, we have extended analysis with plasmid replicons, virulence factors, highly discriminatory genomic...

The lack of new antibiotic classes calls for a cautious use existing agents. Yet, every 10 min, almost two tons antibiotics are used around the world, all too often without any prescription or control. use, overuse and misuse select resistance in numerous species bacteria which then renders antimicrobial treatment ineffective. Almost countries face increased (AMR), not only humans but also livestock along food chain. spread AMR is fueled by growing human animal populations, uncontrolled...

Abstract Objective: To test the hypothesis that perioperative elimination of nasal carriage Staphylococcus aureus using mupirocin ointment reduces surgical-site infection (SSI) rate in cardiothoracic surgery. Design: Unblinded intervention trial with historical controls. Setting: A university hospital, tertiary referral center for Patients: Consecutive patients undergoing surgery between August 1, 1989, and February 1991 (historical control group), March 1991, 1992 (intervention group)....

ABSTRACT Increasing antibiotic resistance in gram-negative bacteria has recently renewed interest colistin as a therapeutic option. The increasing use of necessitates the availability rapid and reliable methods for susceptibility testing. To estimate the proportion of methicillin-resistant Staphylococcus aureus (MRSA) isolates from humans that were sequence type (ST) 398, we surveyed 24 laboratories in 17 countries Europe 2007. Livestock-associated MRSA ST398 accounted for only a small humans; most Netherlands, Belgium, Denmark, and Austria.

In The Netherlands, patients exposed to pigs or veal calves were recently shown be at high risk of methicillin-resistant Staphylococcus aureus (MRSA) carriage. Amphia Hospital (Breda, Netherlands), 32% in this group carry MRSA. This resulted a 3-fold increase the annual MRSA incidence.

ABSTRACT Selective decontamination of the digestive tract (SDD) selectively eradicates aerobic Gram-negative bacteria (AGNB) by enteral administration oral nonabsorbable antimicrobial agents, i.e., colistin and tobramycin. Colistin MICs were determined...

Within days, 1,097 HCW nine hospitals tested; 45 (4.1%) positive. Of six positive HCW, two accounted 38 HCW. The results informed local national risk management.
