Selective digestive tract decontamination (SDD) and selective oropharyngeal (SOD) are infection-prevention measures used in the treatment of some patients intensive care, but reported effects on patient outcome conflicting.
Cyclodextrins (CyDs) are cyclic oligosaccharides, containing a minimum of six D-(+)-glycopyranose units attached by α-1, 4-linkages produced the action cyclodextrin-trans-glycosidase enzyme on medium starch. CyDs somewhat cone-shaped. The outside is hydrophilic and inside cavity hydrophobic in character. If molecule fits entirely or at least partially into cavity, an inclusion complex may be formed. In general, molecules, rather than ones, have higher affinity to CyD aqueous solutions....
Abstract Objective To determine whether valproic acid (VPA), a histone deacetylase inhibitor that showed antioxidative and antiapoptotic properties reduced glutamate toxicity in preclinical studies, is safe effective amyotrophic lateral sclerosis (ALS) using sequential trial design. Methods Between April 2005 January 2007, 163 ALS patients received VPA 1,500mg or placebo daily. Primary end point was survival. Secondary outcome measure decline of functional status measured by the revised...
WHAT IS ALREADY KNOWN ABOUT THIS SUBJECT • Drug–drug interactions (DDIs) may lead to often preventable adverse drug events and health damage. In Dutch community pharmacies approximately 6% of all prescriptions generate a DDI alert. Hospitalized patients be especially at risk, as they are more severely ill multiple medications prescribed simultaneously; however, only limited data available on the frequency nature DDIs during hospitalization. STUDY ADDS university hospital 10% generated alert;...
Objective To describe the frequency and type of potential drug‐drug interactions ( pDDI s) in a general intensive care unit (ICU) to make recommendations improve management these s. Design Retrospective observational study. Setting General ICU tertiary hospital. Subjects All patients admitted for more than 24 hours between May 2009 December 2010 who were prescribed at least one medication. Measurement Main Results Based on G‐Standaard, Dutch national drug database, s identified classified...

Objectives
To determine the safety and efficacy of lithium for treatment amyotrophic lateral sclerosis (ALS) in a randomised, placebo controlled, double blind, sequential trial.
Methods
Between November 2008 June 2011, 133 patients were randomised to receive carbonate (target blood level 0.4–0.8 mEq/l) or as add-on with riluzole. The primary endpoint was survival, defined death, tracheostomal ventilation non-invasive more than 16 h/day. Secondary outcome measures consisted...
Nearly half of all hospitalized infants with respiratory syncytial virus (RSV) lower tract disease (LRTD) are treated (parenteral) antibiotics. The present study was designed to test our hypothesis that the use antibiotics would not lead a reduced duration hospitalization in mild moderate RSV LRTD.Seventy-one patients < or =24 months age virologically confirmed clinical diagnosis LRTD were randomized azithromycin 10 mg/kg/day (n = 32) placebo 39) multicenter, randomized, double-blind,...
Although, drug-drug interactions (DDIs) between potassium-increasing drugs (PIDs) are known risk factors for developing hyperkalaemia, not much is about their and management strategies during hospitalisation. This study examines the frequency of serum potassium measurements hyperkalaemia in hospitalised patients, based on use one or more PIDs, determinants thereof.Adult patients University Medical Centre Utrecht 2006 2008 were included this cross-sectional study. The compared using only PID...
The availability of ready-to-administer (RTA) syringes for intravenous (IV) drugs facilitates rapid and safe administration in emergency intensive care situations. Hospital pharmacies can prepare RTA through aseptic batchwise filling. Due to excess production these sufficient patient their limited (microbiological) shelf-life, waste is unavoidable, which contributes environmental pollution. prefilled sterilized (PFSSs) have much longer shelf-lives than aseptically prepared might contribute...
Abstract Introduction Tight glucose control therapy (TGC) has been implemented to hyperglycemia in ICU patients. TGC may also influence serum potassium concentrations. We therefore investigated the of on both and concentrations associated mortality. Method performed a retrospective analysis including all patients admitted tertiary hospital for 24 hours or more with at least three between 1999–2001 (conventional period), 2002–2006 (implementation period) 2007–2009 (TGC period). Segmented...
In this prospective randomized trial, the efficacy and safety of once-daily administration gentamicin were compared with multiple-daily in infants children. addition, pharmacokinetic variables calculated. Gentamicin therapy was started at a dose 5 mg/kg per day under individual or dosage interval adjustments to achieve target levels. Fifty-two children aged 1 month (postterm) 16 years enrolled. The duration fever from start therapy, percentage decline C-reactive protein (CRP) on 3 treatment,...
(1) Background: Busulfan has been used as a conditioning regimen in allogeneic hematopoietic cell stem transplantation (HSCT). Owing to large inter-individual variation pharmacokinetics, therapeutic drug monitoring (TDM)-guided busulfan dosing is necessary reduce graft failure and relapse rate. As there exists no TDM of administration for HCT Taiwan, we conducted pilot study assess the TDM-dosing Taiwanese population; (2) Methods: Seven patients with from The Koo Foundation Sun Yat-Sen...
High variability in tacrolimus pharmacokinetics directly after lung transplantation (LuTx) may increase the risk for acute kidney injury (AKI) and transplant rejection. The primary objective was to compare pharmacokinetic patients receiving orally versus intravenously early LuTx.
Abstract: We present the case of a 65-year-old patient who was treated with high-dose benzylpenicillin for severe invasive pneumococcal pneumonia, complicated by acute renal failure managed continuous venovenous hemofiltration. After cessation hemofiltration, experienced multiple tonic–clonic seizures. Therapeutic drug monitoring revealed high total serum concentrations benzylpenicillin, identifying it as likely cause neurotoxicity. This study presents first documented concentration associated
Cefiderocol is a broad-spectrum cephalosporin antibiotic and indicated in patients with difficult-to-treat Gram-negative bacterial infections. applied as 2-4-times daily prolonged 3-h infusion. The therapeutic target of cefiderocol suggests that continuous infusion (CI) may be advantageous, since it more likely to achieve 100% time the unbound concentration above minimal inhibitory (MIC). However, limited information on CI has been assessed. We present case critically ill 37-year-old woman...
AbstractBackground:Although the discontinuation of a medication may have important clinical consequences, there is generally much less attention given to surveillance when drug stopped than it started. Objective:To investigate consequences on serum potassium levels discontinuing that increases level (PID↑) and decreases (PLD↓) in patients taking both. Methods:Patients who were hospitalized University Medical Centre Utrecht 2004-2009 using both PID↑ PLD↓ included one these drugs was...

Background and Importance
The availability of ready-to-administer (RTA) syringes for intravenous drugs facilitates rapid safe administration in emergency intensive care situations. However, the preparation these hospital pharmacies via aseptic batchwise filling results significant drug waste due to excess production their limited microbiological shelf-life 31 days, which contributes considerable environmental pollution. RTA sterilised have much longer shelf-lives (up 36 months) than...
