頭皮動静脈奇形はきわめてまれな疾患である．今回われわれは，巨大な頭皮動静脈奇形に対し摘出術を行った症例を経験したので報告する．45歳，男性．幼少時に頭部打撲により右後頭部に皮下血腫を形成し，その後時期は不明だが同部位に軟らかい腫瘤を触れるようになった．今回，家人より増大を指摘され，当科紹介となった．血管撮影では，流入動脈に後頭動脈と浅側頭動脈を，流出静脈に浅側頭静脈と後耳介静脈をそれぞれもつ血管奇形を認めた．頭皮動静脈奇形と診断し，摘出術を施行した．頭皮動静脈奇形の手術においては，栄養血管の結紮のみや不十分な塞栓だけでは，側副路の発達によって残存病変から再発・増大をきたす場合もあり，根治的には全摘出術が望ましいとされている．病変への血流をコントロールし，術中の出血量をできるかぎり減らすこと，また術後の合併症となる皮膚の感染・壊死を予防することが肝要となる．術前の画像検査による流出入血管の十分な評価と，それに基づく手術戦略の立案が求められる．
