ABSTRACT Endobronchial leiomyoma is a rare airway tumour. We report the case of 30‐year‐old female non‐smoker who presented with persistent fever and cough, ultimately diagnosed post‐obstructive pneumonia secondary to an endobronchial leiomyoma. The patient underwent successful cryo‐resection resolution symptoms. This highlights importance considering causes non‐resolving role minimally invasive approach in management
Organising pneumonia (OP) is a distinct pathological pattern characterised by the presence of granulation tissue buds composed fibroblasts and myofibroblasts embedded in loose connective matrix within distal pulmonary airspaces. When OP occurs without an identifiable cause or etiologic context, it termed cryptogenic organising (COP). The diagnosis can be challenging due to its diverse clinical presentations, including idiopathic form various secondary forms associated with underlying...
Abstract A 37‐year‐old woman, 25 weeks pregnant, experienced sudden massive hemoptysis. She had a background history of systemic lupus erythematosus (SLE) and past pulmonary tuberculosis (PTB). Emergency intubation was necessary, bronchoscopy revealed blood pooling in both main bronchi, with active bleeding from the right upper lobe bronchus. Urgent computed tomography (CT) angiography bronchial artery identified source successfully embolized. Antifungal anti‐tuberculous therapy initiated...
Abstract Background Patient comfort during invasive and therapeutic procedures is important. The use of virtual reality (VR) devices flexible bronchoscopy (FB) as a method distraction to increase patient tolerability improve satisfaction has not been investigated. We aim assess the participants undergoing FB with or without VR. Methods This was single-center, open-label study on patients bronchoscopy, randomized into control interventional groups. group received standard care FB. given VR...
Acute COVID-19 usually lasts 4 weeks from the onset of symptoms. We report two cases COVID-19-associated organising pneumonia (OP) occurring beyond acute Both tested positive for SARS reverse transcription–PCR 2 months before presentation with a resolution respiratory The first case presented residual fatigue and worsening exertional dyspnoea. Chest CT revealed an OP pattern. second cough new-onset pleuritic chest pain persistent radiological consolidation. A transbronchial lung biopsy...
Abstract Pelvic tumours are a rare cause of pleural effusion. We describe an approach to case Meigs syndrome with recurrent unilateral A woman in her 60s' presented right‐sided effusion, leading cough and shortness breath. Thoracentesis yielded exudative fluid cytology negative for malignancy. Pleuroscopy revealed inflamed pleura, biopsy was consistent inflammatory changes. The patient's cancer antigen 125 level elevated at 256 U/mL. Given the high suspicion malignancy, computed tomography...
Background Aerobika ® oscillating positive expiratory pressure (OPEP) device promotes airway clearance in many respiratory diseases. However, studies have yet to focus on its effectiveness improving small resistance via impulse oscillometry (IOS) measurement COPD subjects. We aim evaluate the improvement of ( IOS), lung function (spirometry), exercise capacity [ 6-min walking test (6MWT)], symptoms [COPD assessment (CAT)] and severe exacerbation events among subjects using OPEP. Methods This...
Liquid silicone (polydimethylsiloxane) is an inert material that commonly used for cosmetic purpose. Silicone embolization syndrome (SES) can rapidly progress to pneumonitis as a consequence of the injection nonmedical‐grade liquid silicone. We describe case severe complicated with acute respiratory distress and bilateral pneumothorax secondary gluteal augmentation. In this report, we aim discuss our experience approach in managing uncommon SES.
We report a 35-year-old woman who presented with dyspnoea and chest pain for 1 week. High-resolution computed tomography (HRCT) thorax revealed bilateral pneumothoraces diffuse lung cysts. Bilateral intercostal tubes were inserted, there was persistent air leak (PAL) bilaterally. performed an autologous blood patch pleurodesis (ABPP) the left PAL. For right PAL, she underwent successful video-assisted thoracic (VATS) surgery, wedge biopsy, surgical pleurodesis. Histopathology examination...
Abstract Background Current management of poorly draining complex effusions favours less invasive image-guided placement smaller tubes and adjunctive intrapleural fibrinolysis therapy (IPFT). In MIST-2 trial, 10 mg alteplase (t-PA) with 5 pulmozyme (DNase) twice daily for 72 h were used. We aimed to assess the effectiveness safety a modified regimen 16 t-PA DNase administered over 24 in pleural infection. Methods This was single centre, prospective study involving patients drained Primary...
Chylothorax is an uncommon cause of pleural effusion. Recurrent chylous effusions are often resistant to conservative treatment and many need surgical intervention. We report a 69-year-old woman with refractory idiopathic chylothorax medium-chain triglyceride diet intermittent thoracentesis. Lymphangiography lymphoscintigraphy failed identify the site leakage. initiated continuous positive airway pressure (CPAP) 12 h before 48 hours after talc pleurodesis. Chest drain was removed at day 3...
Abstract Indwelling pleural catheter (IPC) is a useful tool for refractory malignant effusions (MPEs). It allows palliation by intermittent symptomatic relief of the effusion and improves quality life. Its use in benign comes mainly from retrospective studies, case series, reports. Lupus common, causes minimal symptoms, usually responds to either steroid therapy or immunosuppressants. Refractory lupus less common treatment may require invasive surgical pleurectomy. We describe 52‐year‐old...
Intrapleural fibrinolytic therapy (IPFT) is one of the treatment options for complex pleural effusion. In this study, IPFT agent used was alteplase, a tissue plasminogen activator (t-PA). This study aims to determine difference in outcome patients with effusion between and surgery terms radiological improvement, inflammatory parameters, length stay, post-intervention complications.A retrospective review treated at Universiti Kebangsaan Malaysia Medical Center from January 2012 August 2020...
Chylothorax is a rare condition caused by pleural effusion resulting from thoracic duct injury. Recurrent chylothorax often resistant to conservative treatment and presents clinical conundrum in its management. Here, we report compelling case of recurrent that persisted despite the administration total parenteral nutrition, octreotide embolisation. The patient eventually required ligation talc pleurodesis, which resulted resolution effusion. Our an illustrative example effective...
Abstract Severe acute respiratory syndrome coronavirus 2 (SARS‐CoV‐2) infection was declared a pandemic on 11 March 2020. We have since seen its fast spread worldwide. A likely contributing factor the lack of symptoms some carriers, making them unaware their risk spreading to other more vulnerable individuals. The important finding has been reported cases infectivity despite symptoms. describe SARS‐CoV‐2 pneumonia patterns in asymptomatic common computed tomography (CT) thorax found are...
Abstract Indwelling pleural catheter (IPC) has revolutionized the management of malignant effusion (MPE). IPC is relatively safe, although complications can occur. We report a 53‐year‐old woman with stage IVA lung adenocarcinoma and recurrent MPE. Two months post insertion, was blocked residual presence new loculations. Attempts to restore patency six doses intrapleural (IP) streptokinase failed. She referred our centre for further management. used single dose 2.5 mg IP alteplase which...
Background Bronchial asthma is characterised by chronic inflammation of both large and small airways. Extra-fine formulation inhaled corticosteroid (ICS) can achieve a higher peripheral lung deposition leading to improved control. We investigated the effect extra-fine fine particle size ICS with airway resistance severity symptoms in asthmatic patients. Methods Prospective interventional study outpatient asthmatics Universiti Kebangsaan Malaysia Medical Centre (UKMMC). Subjects were grouped...
