Abstract No reports of vasovagal reflex activity during robotic‐assisted laparoscopic hysterectomy (RALH) exist. We present a case woman who underwent RALH for uterine myoma with adenomyosis. A lack tactile feedback and traction force sensor create unique risks robot‐assisted surgery. Anesthesiologists should be aware these risks.
周産期心筋症(peripartum cardiomyopathy, PPCM)は,心疾患の既往のない女性が妊娠最終月から産褥期5ヶ月の時期に心不全を発症する疾患である。非常に稀な疾患ではあるが,低心機能が残存する可能性も高く,心臓移植が必要になることもある重篤な疾患である。症例は4回経産の40歳女性。2日前から続く呼吸困難で当院を妊娠36週6日に受診した。心疾患の既往はなく,胸部X線写真で両側肺うっ血と胸水貯留を認め,経胸壁心エコー上左室駆出率18%であり,PPCMと診断され緊急帝王切開術を施行した。術後ICUで非侵襲的陽圧換気,利尿薬,ドブタミンなどで治療し,POD 2にICUを退室した。周術期管理が奏功し,無事にPOD 23に退院したが,PPCM発症から6ヶ月後に小脳梗塞を発症した。持続する低心機能,産褥期の凝固能亢進の関与が考えられた。
Many acute and subacute complications of coronavirus disease 2019 (COVID-19) have been reported. However, the recovery process in severely ill patients is not clear.
