周産期心筋症（peripartum cardiomyopathy, PPCM）は，心疾患の既往のない女性が妊娠最終月から産褥期5ヶ月の時期に心不全を発症する疾患である。非常に稀な疾患ではあるが，低心機能が残存する可能性も高く，心臓移植が必要になることもある重篤な疾患である。症例は4回経産の40歳女性。2日前から続く呼吸困難で当院を妊娠36週6日に受診した。心疾患の既往はなく，胸部X線写真で両側肺うっ血と胸水貯留を認め，経胸壁心エコー上左室駆出率18％であり，PPCMと診断され緊急帝王切開術を施行した。術後ICUで非侵襲的陽圧換気，利尿薬，ドブタミンなどで治療し，POD 2にICUを退室した。周術期管理が奏功し，無事にPOD 23に退院したが，PPCM発症から6ヶ月後に小脳梗塞を発症した。持続する低心機能，産褥期の凝固能亢進の関与が考えられた。
