Pulmonary embolism remains a leading cause of cardiovascular mortality. Presentation and outcomes are variable among patients require rapid risk stratification for assessment prognosis, as well selection appropriate treatment. Over the past several decades, different models parameters have become available to assess classify pulmonary into categories. Some may be candidates early discharge or complete outpatient treatment, while some invasive diagnostics intensive monitoring. In this review,...

Cutaneous fistula as a clinical presentation of intracardiac abscess the right side is such an unusual occurrence that it has not until now been reported in English-language medical literature. We present rare case right-sided infective endocarditis caused by Achromobacter xylosoxidans which recurrent infection presented sternal wound discharge. The was found to have origin and successfully managed radical débridement on cardiopulmonary bypass.

Extracorporeal membrane oxygenation (ECMO) has become an integral treatment option for patients as a bridge to transplant, management of post cardiotomy cardiogenic shock, and rescue after cardiopulmonary arrest. Significant strides in ECMO technology cannot, however, replace the importance maintaining following comprehensive safety checklist. We herein report case massive air entrainment from inadvertently disconnected port central venous catheter (CVC) neck which culminated airlock...

Patients with cardiac and thoracic trauma remain one of the most difficult presentations to diagnose treat in an emergency room setting. Here, we present our series four cases varied presentations, including lung, vascular, diaphragmatic injuries that were managed successfully. We further review manifestations trauma, contusions, rupture, pericardial injury, valvular injuries; lung injury. The sheer complexity anatomical structures within thorax makes it utmost importance timely...

Abstract Background and Aim of Study: Right-sided aortic arch is a rare congenital anomaly for which different surgical approaches have been reported. This study reviewed our experience with several techniques. Methods: We retrospectively 17 patients undergoing right-sided repair at the Cleveland Clinic from 2001 to 2010. Computed tomographic angiograms aorta its branches were correlated patient presentation approach. Results: Fourteen had type II right aberrant left subclavian artery....

Limb-related complications are major contributors to extracorporeal membrane oxygenation-associated complications. Early detection of limb ischemia and or compartment syndrome is paramount minimizing the adverse effects. With absence pulsatile flow, bedside Doppler examination distal arterial waveforms an impractical unreliable method monitoring perfusion. We describe a new application near-infrared spectroscopy tissue oxygenation for early in oxygenation.
