症例は58歳,男性。新型コロナウイルス感染症とARDSに対して人工呼吸,veno venous extracorporeal membrane oxygenation(VV-ECMO)管理となった。酸素化の改善が得られたものの,低コンプライアンスが改善せず,カテーテル関連血流感染の治療に難渋したため,来院第98病日に通常のVV-ECMOから透析用カテーテルと遠心ポンプを用いたextracorporeal CO2 removal(ECCO2R)に移行した。分時換気量の低下,食道内圧の変動の低下から,ECCO2Rによる二酸化炭素除去は呼吸努力を軽減し,肺保護換気戦略を可能にしたと考えられた。ECCO2Rは二酸化炭素の除去のみを目的とした体外循環であるが,本邦には専用のデバイスが存在しない。本症例では透析用カテーテルと遠心ポンプを併用した方法で安全に施行できた。また,ECCO2Rの適応は定まっていないが,長期ECMO後の離脱困難症例に対して有効な可能性がある。
