背景.肺カルチノイドは原発性肺悪性腫瘍の1~2%を占め,稀にadrenocorticotropic hormone(ACTH)産生を伴う.症例.42歳,女性.1年前より体重増加,満月様顔貌を自覚しており,2ヶ月前からの腰背部痛を主訴に当院を受診.特徴的な身体所見や,ACTH及びコルチゾールの上昇を認め,他の内分泌学的精査と合わせてCushing症候群と診断された.胸部CTで左肺上葉に腫瘍を認め,異所性ACTH産生腫瘍が疑われたため,左上葉切除術,リンパ節郭清術を施行した.病理診断は定型カルチノイドで,大動脈下リンパ節に転移を認めた.また,免疫染色でACTHが陽性であり,ACTH産生肺定型カルチノイド,pT1bN2M0,stage IIIAと診断された.術後,ACTH及びコルチゾールは一過性に低下したが,速やかに再上昇し,高値が遷延した.術後精査でその原因を説明する所見はなく,トリロスタン投与後,切除7ヶ月後にいずれも正常範囲まで低下した.現在,術後3年であるが,再発や症状増悪は認めていない.結論.特異的な術後ホルモン濃度の推移を辿ったACTH産生肺定型カルチノイドの1例を経験した.

症例は53歳,男性.幼少期のてんかん,精神発達遅滞の既往あり.嘔気,嘔吐を主訴に前医を受診.腸閉塞の診断で2日間保存的に加療されたが,症状の改善に乏しく,当院へ紹介となった.胸部単純X線写真では,腸管の拡張とniveau像,腹部造影CTでは著明な腸管拡張,右下腹部で上腸間膜静脈を中心に腸間膜組織が渦巻き状に巻き込まれる腫瘤像(“whirl sign”)を認め,腸軸捻に伴う腸閉塞の診断で手術治療の方針となった.開腹すると,結腸全体の著明な拡張,盲腸およびS状結腸の捻転を認め,結腸亜全摘術を施行した.術後は麻痺性イレウスが遷延し,術後約3カ月で退院した.腸軸捻は腸間膜と共に腸管が捻転する病態であり,多くは単独箇所で生じ,複数箇所の捻転が同時に発症することは稀である.今回,盲腸軸捻・S状結腸軸捻を同時に発症した非常に稀な症例を経験したので,文献的考察を加えて報告する.

症例は71歳,男性.食思不振を主訴に前医を受診し,食道胃接合部癌と診断された.精査中に全身に多発する緊満性水疱,びらんが出現し,精査目的に当院へ紹介.皮膚生検で水疱性類天疱瘡と診断され,蛋白漏出に伴う低蛋白血症も伴っていた.臨床経過から悪性腫瘍のデルマドロームとして水疱性類天疱瘡の発症が疑われた.皮膚症状のコントロール目的にステロイド投与および栄養介入後,下部食道-噴門側胃切除術を施行した.病理診断は乳頭腺癌,pStage III Aであった.現在,術後約4カ月であるが,再発は認めていない.また,皮膚症状や低蛋白血症は改善傾向である.水疱性類天疱瘡と悪性腫瘍の合併はしばしば報告されているが,その関連に一定の見解が得られていない.本症例はステロイドへの反応も良好であったが,腫瘍切除後に皮膚症状はさらに改善した.強い関連が示唆された症例であり,臨床経過を治療戦略とともに考察する.

光線力学的治療(photodynamic therapy: PDT)は中心型早期肺癌に対する標準的な治療法の一つであり,その治療成績向上のためには,腫瘍の範囲を正確に診断し,適切にレーザー照射を行うことが重要であるが,一部の内視鏡システムは,光感受性物質であるタラポルフィンナトリウム注射後の腫瘍進展範囲の評価に不向きである点や,レーザー光により色情報が担保できず,適切なレーザー照射に苦慮するなどの課題があった.今回は,同時式内視鏡システムELUXEO 7000®(富士フイルム株式会社)を用いてPDTの治療成績向上に向けた工夫を検討した.
