Fecal culture for isolation and identification of Shigella may take days. The BioFire FilmArray Gastrointestinal (GI) panel (bioMérieux, France) is a PCR-based assay that detects enteric pathogens including /enteroinvasive Escherichia coli (EIEC) in about an hour.
Acupuncture is a nonpharmalogical treatment modality that used to treat pain. Prior research demonstrates pediatric patients tolerate acupuncture well and may experience significant pain relief. The objectives of this study were (1) explore the experiences emergency department (ED) physicians as they 2 procedures, Battlefield Four Gates (2) describe factors impacting feasibility implementation in ED setting. This qualitative individual semistructured interviews with medicine who had completed...
Foreign body ingestion is a common pediatric complaint. Two case reports describe intestinal obstruction in children from an of single superabsorbent water ball, requiring surgical removal. We nonsurgical management asymptomatic child who ingested approximately 100 beads. Because the risk for subsequent obstruction, patient was admitted whole bowel irrigation. This report first describing use irrigation with multiple beads ingestion.
Multiplex molecular diagnostic panels have greatly enhanced detection of gastrointestinal pathogens. However, data on the impact these tests clinical and patient-centered outcomes are limited.
The evaluation for children with bruising may be affected by the specialty to which they are referred. We conducted a 3-year retrospective review of subjects referred Child Abuse Pediatrics (CAP) or Pediatric Hematology identify characteristics associated referral each and compare diagnostic evaluations diagnoses based on specialty. Of 369 subjects, 275 were CAP 94 Hematology. Clinical exam findings similar in both groups. referrals significantly more likely have laboratory evaluations....
Multiplex molecular diagnostic panels have greatly enhanced detection of gastrointestinal pathogens. However, data on the impact these tests clinical and patient-centered outcomes are limited. We conducted a prospective, multicenter, stepped-wedge trial to determine multiplex testing at five academic children's hospitals in children presenting ED with acute gastroenteritis. Caregivers were interviewed enrollment again 7-10 days after symptoms, risk factors, subsequent medical visits, family...
Among 111 children presenting with bloody diarrhea in a multicenter study of molecular testing US emergency departments, we found viral pathogens 18%, bacteria 48%, protozoa 2%, and no detected 38%.
We used molecular testing to examine the causes of bloody diarrhea in a multicenter study pediatric gastroenteritis. Pathogens typically associated with were detected less than half cases, and inappropriate antibiotic use was common, supporting stool patients diarrhea.
To evaluate the likelihood of abuse for various fractures, we aimed to compare prevalence concomitant suspicious injuries (CSIs) in subjects with fractures presumed be low risk those non-low-risk (aim 1) and low-risk identified on skeletal survey (SS) 2).Subjects included toddlers 9 23 months age presenting a children's hospital system fracture having an SS completed as well who had any concern 2). For aim 1, performed 5-year retrospective case-control study. Low-risk were defined extremity...
Abstract Background Infectious gastroenteritis is a major cause of morbidity and mortality among children worldwide. While most episodes are self-limiting, for select pathogens such as Shigella Campylobacter, etiological diagnosis may allow effective antimicrobial therapy aid public health interventions. Unfortunately, clinical predictors not well established based on small studies using bacterial culture identification. Methods We used prospectively collected data from multi-center study...
Acute viral gastroenteritis is one of the leading causes diarrheal diseases. The FilmArray GI Panel a PCR based assay that detects 22 different enteric pathogens including five viruses (Adenovirus F 40/41, Astrovirus, Norovirus GI/GII, Rotavirus A, and Sapovirus (I, II, IV, V)) in an hour. epidemiology management acute described. Children with were prospectively enrolled at emergency departments geographically pediatric facilities during 2015–2016. Stool specimens collected tested by Panel....

BACKGROUND: The opioid crisis catalyzed a national drive to better utilize non-pharmacologic pain management. To our knowledge, no study has examined staff, parent, or patient perspectives on acupuncture implementation in pediatric emergency departments (PED). OBJECTIVE: understand and attitudes barriers related PED acupuncture. METHODS: We conducted 2 studies at PEDs within single children’s health system (125,000 visits/year). In Study 1, English-speaking patients (12-18 years), parents,...
