2002年から2008年までに当科で手術加療を行った上歯肉癌31例を対象として臨床病理学的検討を行った。原発巣の切除術式は1.粘膜切除,2.歯槽骨切除,3.上顎部分切除,4.上顎(亜)全摘が選択されそれぞれ2例,4例,19例,6例であった。病理診断では疣状癌と高分化扁平上皮癌で31例中19例と高分化型が約60%を占めていた。分化型毎に上顎骨浸潤の有無を比較すると,高分化型では57.9%(11/19),中分化型では62.5%(5/8),低分化型では100%(2/2)で高分化型でも骨浸潤は高率に認められた。5年無再発生存率は56.2%だったが,12例再発したうち8例が救済可能(Salvage率66.7%)で5年粗生存率は83.9%と良好な結果であった。再発形式では頸部リンパ節のみの再発は5例でうち1例が救済不能であったが,原発巣再発をきたした6例は3例しか救済できず初回治療時の切除範囲の決定が重要であると思われた。
