本邦において破傷風は減少傾向だが,年間100例以上の発生報告があり,死亡率は10〜20%に上る重篤な疾患である.最も症状が強くなるⅢ期は苦痛緩和を目的とした深鎮静が必要となりリハビリテーションが進まないが,回復期といわれるⅣ期まで安静を続けると廃用が進行する.つまり,重症患者管理において重要なリハビリテーションを破傷風患者に行うのが困難なことが臨床上の問題である.今回われわれは人工呼吸管理を要した重症全身性破傷風のⅢ期から,プロポフォールをデクスメデトミジンに変更した鎮痛鎮静管理によって覚醒を維持した状態でリハビリテーションを行うことが可能となり,機能障害なく退院した症例を経験したため報告する.
