【目的】本研究の目的は,エンド・オブ・ライフ(EOL)ケアに関する看護実践測定尺度の開発を目指し,その信頼性と妥当性を検討することである.【方法】4病院の,前年度死亡患者数10名以上の病棟に勤務する看護師を対象に無記名自記式質問紙調査を行った.【結果】探索的因子分析の結果,18項目からなる1因子構造の尺度が同定された.Cronbachのα係数は0.95であり,十分な内的一貫性が認められた.EOLに関する教育の有無などによる尺度得点の比較により,既知集団妥当性が示された.【考察】今回開発した看護師のEOLケア実践評価尺度は,内的一貫性と既知集団妥当性を有することが確認された.本尺度の活用により,看護師のEOLケア実践やEOL教育効果の測定が可能となる.

近年アドバンス・ケア・プランニング(advance care planning:ACP)の有用性に関するエビデンスが蓄積されてきた.しかし,膵癌・胆道癌患者と家族に対して,具体的にどのようなACPを行うのがよいかについてはわかっていない.今回,ビデオを活用してACPのきっかけ作りを行うことを目的に,膵癌・胆道癌教室のプログラム(授業)を開発した.International Patient Decision Aids Standardsの手順に基づき,患者,家族や多職種の医療者のフィードバックをもとに,5分のビデオを含む計10分間の授業を作成した.その過程で,3回の膵癌・胆道癌教室で本授業を行い,実施可能性を確かめた.本稿では,本授業の開発過程を詳述する.今後,多施設でACPの患者教育を行うにあたり,ビデオを活用した授業は簡便で有望と考えられる.
